Welcome to PlanforYourHealth.com ("site"), a website designed by Aetna and the Financial Planning Association ("we"). We appreciate your interest in us.

Collecting and Using Personal Information

When you visit and navigate our site and when you communicate with us via our site, we will not collect personal information about you unless you provide us that information voluntarily. From time to time, we may request personal information from you at our site in order to deliver requested materials to you, respond to your questions, or deliver a product or service. Any non-public personal information that you may provide via our site will be used solely for the purpose stated on the page where it is collected. We will not sell, license, transmit or disclose personally identifiable information outside of Aetna, the Financial Planning Association and its affiliated companies unless (a) expressly authorized by you or (b) required or permitted by law. In all cases, we will disclose the information consistent with applicable laws and regulations and we will require the recipient to protect the information and use it only for the purpose it was provided. By "personal information," we mean data that is unique to an individual, such as a name, address, social security number, or telephone number.

Security of Information

The personal information that you submit to us is stored in our secure database behind an electronic firewall. PlanforYourHealth.com carefully safeguards your personal information. However, like other Internet sites, we cannot guarantee that it is completely secure from people who might attempt to intercept transmissions over the Internet or breach our security measures.

Similarly, we cannot ensure the privacy of e-mail communications to and from our site because they are not encrypted and, therefore, are not secure. These communications, like most Internet e-mail, could be intercepted without our or your knowledge and permission while in transit to or from our site. This includes e-mail you send to us through our various mailboxes, our free e-mail newsletters and information e-mailed from our site to others using features such as sending tell-a-friend items. If you are concerned about your privacy, we suggest that you do not use e-mail to communicate information to us that you consider sensitive.

When you use our "tell-a-friend" function, we ask you to provide e-mail addresses for your friends and relatives. We use the e-mail addresses to send a one-time "tell-a-friend" email to those individuals. The e-mail addresses you provide will not be used for any other purpose, unless those individuals choose to register or otherwise be in communication with us.

Registration

You are always welcome to use PlanforYourHealth.com without registering or providing any personal information. However, in order to receive certain services, including our free e-mail alerts/reminders, e-mail newsletters and tell-a-friends, and in order to register for the Navigating Your Health Benefits For Dummies tool, you must complete a registration form. When you register for these features, we will clearly indicate what registration information is required, what is optional and how that information may be used.

Here is a summary of our registration requirements:

When you register for free e-mail alerts or e-mail newsletters, we require the following personal information: e-mail address, first and last name, zip code and year of birth. We also ask you to agree to the terms of the privacy policy.

We use this information in aggregate form only. In other words, we combine the information with data collected from other individuals to produce anonymous, aggregated statistical information. We collect and use demographic information, such as the percentage of female visitors or the number of visitors of a certain age, in order to improve the content of our site. We also may share this anonymous, aggregated information, which does not identify individuals, with partners or other third parties. In instances where third parties may receive anonymous, aggregated user information collected on our site, our agreements with third parties provides that the parties will not use the anonymous, aggregated user information to identify individual users or sell, disclose or provide access to any user information collected on our site that would identify any individual.

Visitors can also register to receive one free copy of the Navigating Your Health Benefits For Dummies per United States address via postal mail. Visitors will be asked to provide their first and last name, mailing address, and how you heard about the guide.

When you choose to tell a friend about the free guide, we will ask for your e-mail address and the address of the person receiving the tell-a-friend e-mail. The e-mail address of the card's recipient will not be used for any purpose other than to send the tell-a-friend.

We treat any personal information you give us as confidential. We do not analyze your personal information nor do we collect specific information about your use of our site. We will send you only information that you request.

Changing or Deleting Your Personal Information

To have your registration information removed, send your request by e-mail to feedback@planforyourhealth.com. Instructions for unsubscribing from e-mail are located at the bottom of each e-mail. Click the link given and uncheck each subscription you wish to cancel. If you registered to receive Navigating Your Health Benefits for Dummies and multiple e-mail alerts/reminders/newsletters and also want that information removed, please note that in your request. Please inform us if you used different e-mail addresses when registering for Navigating Your Health Benefits for Dummies or e-mail alerts/reminders. You will receive a confirmation from us.

Unsubscribing from our e-mail newsletters does not entirely delete your personal information from our system, even if you unsubscribe from all e-mails. We will delete your personal registration information from the active and searchable areas of our system at your request. However the data will be retained in our archives and will not be deleted.

Contacting Us

We welcome your comments or questions about our site and have provided e-mail boxes for that purpose. We will share your comments and questions with the Aetna and Financial Planning Association representatives most capable of addressing your questions and concerns. To protect your privacy, please do not use e-mail to communicate information to us that you consider confidential. If you wish, you may contact us instead via non-cellular telephone at the numbers provided at various locations on our sites or, in the case of our Aetna health plan members, at the Member Services toll free number that appears on your identification card.

Other Information: "Cookies" and IP Addresses

You should also be aware that when you visit PlanforYourHealth.com, we collect certain information that does not identify you personally, but provides us with "usage data," such as the number of visitors we receive or what pages are visited most often. This data helps us to analyze and improve the usefulness of the information we provide at this site.

"Cookies" Like most commercial website owners, we may also use what is known as "cookie" technology. A "cookie" is an element of data that a website can send to your browser when you link to that website. It is not a computer program and has no ability to read data residing on your computer or instruct it to perform any step or function. By assigning a unique data element to each visitor, the website is able to recognize repeat users, track usage patterns and better serve you when you return to that site. The cookie does not extract other personal information about you, such as your name or address.

PlanforYourHealth.com uses cookies for user convenience. The cookie will tell our system certain information so you don't have to fill in the same information each time you visit us. An example of this is our tell-a-friend feature.

Most browsers provide a simple procedure that will enable you to control whether you want to receive cookies by notifying you when a website is about to deposit a cookie file. If you choose to turn off cookies, you can still access all areas of our site. Once the cookie feature is turned off, you may have to "refuse" a cookie each time you encounter one while browsing our site.

IP Addresses An Internet Protocol (IP) address is a number that is automatically assigned to your computer by your Internet Service Provider (ISP) whenever you access the Internet. This number may be the same each time you access the Internet or it may change. Like other Internet sites, PlanforYourHealth.com uses IP addresses to collect aggregate information about numbers of visitors and site usage. The address is logged on our server when your computer requests information from PlanforYourHealth.com. However, to allow users to access our site anonymously, your IP address is not linked with personal information about you. PlanforYourHealth.com reserves the right, in rare instances, to use IP addresses to identify individuals who threaten our site, service or customers.

Surveys, Questionnaires and Polls

When you respond to surveys, questionnaires or polls on our site, we may, at times, ask you for demographic information such as your gender or age. This information is collected only as anonymous, aggregated information and is used for statistical purposes. Otherwise, we do not collect or store your personal responses.

Contests

When we run a contest, we may ask for personal information when you enter the contest. If so, we will describe in each instance why we are asking for that personal information and how we plan to use that information.

Children

We will not knowingly allow children under the age of 13 to register on our site for any purpose or to access any features that require registration. We require visitors to indicate their year of birth to register. If we learn that we inadvertently have collected identifying personal information from a child under the age of 13, we will promptly delete that information from our system.

Linking to Other Sites

From time to time, our site may provide links to other websites, not owned or controlled by Aetna or the Financial Planning Association, that we think might be useful or of interest to you. We cannot, however, be responsible for the privacy practices used by other website owners or the content or accuracy of those other websites. Links to various non-Aetna and non-FPA websites do not constitute or imply endorsement by Aetna or FPA of these websites, any products or services described on these sites, or of any other material contained in them. At times, clicking on a hyperlink within an article will bring up information on another site without warning. In these cases, however, the information will appear in a second, smaller "pop-up" window and the PlanforYourHealth.com page will remain clearly visible on your computer screen.

Changes to This Statement

Aetna and the Financial Planning Association may change this Statement from time to time without notice. This Statement is not intended to and does not create any contractual or other legal rights in or on behalf of any party.
