Receiving statements
Under most health insurance plans, an Explanation of Benefits (EOB) form is provided directly to you to explain how a health benefits claim was paid. This form shows you the amount of the claim, the amount paid by your plan, and the amount you are responsible for paying. In addition to claims payment information, the EOB often includes information on the appeals process. EOBs are often mailed and also may be available online.
